The Witcher 3's second and final expansion, Blood and Wine, is supposed to launch tomorrow, May 31, but season pass holders on Xbox One are in luck. If you head to The Witcher 3: Wild Hunt on My Games and Apps you'll see that the expansion is ready to install.
