ESRB (Entertainment Software Rating Board) have taken a closer look at Metal Gear Solid V: Ground Zeroes. No surprise that the game contains violence and strong language, but the inclusion of "sexual violence" is more intriguing. ESRB notes that an audio file is the reason:

"The game includes an audio file in which a female character is sexually assaulted by male characters; while there is no visual depiction, sounds of ripped clothing and struggle can be heard."

Metal Gear Solid V: Ground Zeroes has already caused some controversy over female character Quiet's lack of clothing.
